The EPC plan, now often referred to as Chronic Disease Management (CDM) plans, is designed for individuals with chronic medical conditions. If your General Practitioner (GP) determines that you qualify—typically for ongoing conditions requiring multidisciplinary care—you may be eligible for up to five allied health visits per calendar year, including physiotherapy.

• Rebate Amount: The Medicare rebate deducts approximately $55 from the cost of each session.
  
• Gap Payment: The remaining gap depends on the length and type of session you choose. We offer flexible session durations to suit your needs, and our team can provide a quote based on your selected service.
  
• Eligibility: These care plans are not available to everyone; they are specifically for those with chronic conditions as assessed by your GP. Common examples include arthritis, diabetes, heart disease, or other long-term health issues that benefit from ongoing management.
  

How to Access Medicare Rebates

To take advantage of the EPC rebate:

1. Consult your GP to discuss your condition and request an EPC plan referral if appropriate.
